A copy of all complaints against physician James B. Tandy, license numbers 01030277A and 01030277B.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

According to our records, there were no litigation files on James B. Tandy, M.D., to be copied. Nor has there been any action against his license on behalf of the Medical Licensing Board. Dr. Tandy - Indiana medical license number 01030277A was issued August 21, 1980 and CSR 01030277B was issued on September 30, 1986 and both expired on October 31, 2019. He graduated from Indiana University in 1980.
