Pursuant to the Indiana Access to Public Records Act, I hereby request the following records:

Any and all documents relevant to all Indiana abortion clinic license requests but were denied the right to perform abortions within the last fifteen years (January 2000 to Current).

Any and all documents relevant to all Indiana abortions clinics allowed the license to perform abortions within the last fifteen years (January 2000 to Current).

I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service atMuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I would request your response within ten (10) business days.

The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, and is not being made for commercial usage.

This email is the response to the written request for copies of public records that you submitted to the Indiana State Department of Health on June 18, 2015.
According to your email (see below the signature block), there may have been multiple attempts at submitting the request. However, from the email, it cannot be determined to whom or what agency the requests where submitted.

Nevertheless, the agency is reviewing agency records to determine whether they contain any documents that you have requested.

We plan to provide you with an additional response to your request on or before July 18, 2015. If there are documents that are responsive to your request, you may be required to pay a copy fee of $0.10 per page before we will photocopy them. Upon initial review of our records, the estimated volume for records that must be printed for redaction (due to inclusion of confidential information) is in the range of 6500 to 7000 pages which would result in an estimated cost of $640.00 to $700.00. Records containing confidential information cannot be redacted electronically and therefore must be printed.

This estimate does not include records held in electronic form without confidential information. The records held electronically without confidential information will be provided at no cost.

Please advise how you wish to proceed.
1. Only electronic records without confidential information (free).
2. Only redacted records that contained confidential information ($640 - $700 estimated cost).
3. Electronic records without confidential information AND redacted records that contained confidential information.
4. Withdraw the request for public records.
