Pursuant to the Illinois Freedom of Information Act., I hereby request the following records:

I request that a copy of the following documents [or documents containing the following information] be provided to me that pertains to Stateville (parent facility and NRC) Correctional Center:

Past years' master menus (2017 - present)
Yearly/Monthly/Weekly Menu Planning Guides
Diets for Health, including those diets of confined people with Diabetes (insulin and non-insulin dependent), "Hyperlipidemia," Hypertension, "Morbid Obesity," "Obesity"
Miscellaneous Diets/Menu Plans (Non-Therapeutic and/or Therapeutic)
Ascetic or religious diets
Diets/menus/meal plans for different units inside the facility (e.g. Administrative Segregation menu, NRC menu, COVID-19 wing menu)

Please provide information that's most recent, and if unavailable any older information including year 2017 - present.

Please include information about food distributors and any menu pricing or cost information.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes. In order to help to determine my status to assess fees, you should know that I am affiliated with a community organization and this request is being made out of concern for public health.

In the event that there are fees, please inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically.
