Contract Section 2.2.1 “State of Illinois Functions and Duties”

Pursuant to the Illinois Freedom of Information Act (5 ILCS 140/1 to 11), I hereby request the following records:

All materials and documentation relevant to the execution and performance of IDOC's contract with Wexford Health Sources, Inc, as described and required by the following clauses:

● All documents concerning the establishment and monitoring of contractual performance targets (Contract Section 2.2.1.13).

● All audit reports, target result reports, and penalty documentation generated as a result of this contract stipulation.(Contract Section 2.2.1.14)

● Copies of all vendor contract reconciliations and monetary adjustments to the vendor. (Contract Section 2.2.1.15)

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

Thanks so much for your quick response to our request. I believe the following wording, submitted in replacement of the wording you cite in your letter, significantly narrows the scope and time frame of the request:

Concerning IDOC's contract with Wexford Health Sources, Inc, please furnish the following documents created for the two years ended December 31, 2015:

● All documents concerning the establishment and monitoring of contractual performance targets created in compliance with Contract Section 2.2.1.13.
● All audit reports, target result reports, and penalty documentation created in compliance with Contract Section 2.2.1.14.
● Copies of all vendor contract reconciliations and monetary adjustments to the vendor created in compliance with Contract Section 2.2.1.15.
