To Whom It May Concern:
Pursuant to the Illinois Freedom of Information Act., I hereby request the following records:
Copies of all incident reports for fatal collisions involving a vehicle between January 1, 2019 and October 31, 2020, in Cook County in which one or both parties is listed as a courier or delivery driver.
State law requires the Chicago Police Department to collect information about fatal collisions per the format specified in the Traffic Crash Report, SR1050.
If possible, please provide an inventory of the requested records in a machine-readable format, such as a CSV file or Excel spreadsheet.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.
Please be advised that, pursuant to FOIA Section 7(1)(a), traffic crash reports in the possession of the Department are exempt from inspection and copying pursuant to Illinois Vehicle Code, 625 ILCS 5/11-408 and 5/11-412. Therefore, the Department is precluded by law from furnishing copies of crash reports to any member of the public. Under these provisions of the Vehicle Code, these accident reports are for the confidential use of the Department and federal, State and local agencies conducting safety studies. See, Arnold v. Thurston, 240 Ill.App.3d 570 (1992).
