A copy of all non-exempt information collected on COVID-19 PUIs that has been submitted into REDCap, based on the IDPH Interim 2019 novel coronavirus patient under investigation form, or similar forms. While the requested information may initially seem difficult and disparate, please note that all of this should be entered into the same form, and will be easily exportable. The information, in full, is as follows:
