Procedures re: reporting inmate injury/death at public and privately-run correctional facilities (Idaho Department of Correction)

The records custodian will review each request and determine if the documents requested are available and not exempt from disclosure. Record requests shall be submitted for existing records, not research projects.

Fees are as follows:
· $9.25 if the record must be retrieved from state storage
· Postage (if not prepaid)
· Cost of labor of person gathering the documents if more than 2 hours is required, OR -
· The request is for more than one hundred (100) pages of paper records; or the request includes records from which exempt information must be redacted regardless of time.
· Copies in excess of 100 pages cost $ .10 per page.
· When available, electronic copies will be provided.

Pre-payment may be required.

All procedures and associated forms (blank) relevant to the reporting of inmate injuries and death at both public and privately-run correctional facilities, should differences apply.
I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
