Police Incident Report for Driver Michael Lewis

To Whom It May Concern:

Pursuant to the Texas Public Information Act, I hereby request the following records:

On 08/11/19, our driver was rear-ended by another motorist on Hwy 59 N at approximately 12:20 a.m. The office wrote up a incident report but missed some crucial details. Since then our organization has been subject to ligitation based on the narrative explained by the officer and additional details that were entered incorrectly. I personally met with the officer and his Sergeant to request the updates so the report reflects properly but to no avail, it's been nearly another 3 weeks and 2 months since the initial incident.
