Pursuant to Hawaii's Uniform Information Practices Code, I hereby request the following records:

Any licensing application documentation related to state accreditation for a Substance Abuse Treatment Program, submitted to the Alcohol and Drug Abuse Division during the years of 2006-2011, and received from the non-profit business entity known as "Narconon Hawaii" dba "Narconon Hawaii New Life Detoxification Program" or "Narconon Fresh Start." Also, for any submissions for accreditation found, please include any documentation sent to the applicant from the DOH defining the outcome, determination or findings of the department's review.

According to public records, this entity was the beneficiary of various grants and fundraising efforts for opening an inpatient treatment facility on a property leased from the Department of Hawaiian Home Lands during this time period, which was located at: 91-1011 Lexington St, Kapolei, HI 96707

I also request that, if appropriate, fees be waived as I believe this request is in the public interest due to the widely reported celebrity fundraiser for Narconon Hawaii to open a Scientology-based drug rehab at this location. This fundraiser was hosted by Kelly Preston and John Travolta on 24-May-2007 and received state government recognition. To this day, Narconon Hawaii falsely claims they have a drug rehab facility in the state of Hawaii and promotes this location as being the facility Kelly Preston helped open. Therefore, the public interest will be greatly served by releasing the documents I am seeking to explain how a state owned property was leased to a controversial drug rehab that never materialized. Additionally, the requested documents will be made available to the general public, and this request is not being made for commercial purposes.
