To Whom It May Concern:
Pursuant to Hawaii's Uniform Information Practices Code, I hereby request the following records:
All reports of complaints and formal/informal grievances from Saguaro Correctional Facility---including but not limited to those submitted via Form PSD 8215---accepted for formal processing and entered into the Corrections Information Management System (CIMS), currently referred to as Offendertrak.
Please redact personally identifying information as necessary but include all other segregable information, including the date the grievance was filed, the nature of the grievance, and its current status or resolution.
This request is being made as part of an ongoing investigation. The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 business days, as the statute requires.

In researching this matter further, if you are seeking information about grievances filed about complaints at SCC, then PSD is not in possession of these documents. Only if an inmate is grieving a PSD issue (lost property in transit, issue prior to SCC etc) then we may have a grievance on the subject.
However, the grievances are not logged by facility for OOS (out of state inmates). They are a lumped together on offendertrak and would require individual paper file screening etc. = possibly not cost effective.
Is there any specific subject matter that you are concerned with?
