PIA request for death records

To Whom It May Concern:

Please consider this a Public Information Act request for case records dating Jan. 1, 2019, to Dec. 31, 2019. Please respond with the following information for each case:
- case number
- date of death
- race
- ethnicity
- age
- gender
- location (e.g. zip code of decedent)
- cause(s) of death (indicating if COVID related)
- any available occupational data
- any other non-exempt information contained in case records

The requested documents will be made available to the general public, and this request is not being made for commercial purposes. In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically by e-mail attachment, in spreadsheet format if possible.
