Pursuant to the Oregon Public Records Law, I hereby request the following documents:

All information, including without limitation all records, documents, incident reports, tickets, traffic stops, infractions, violations, complaints, domestic violence calls, 911 calls, DUI records, arrest records, convictions, pleas, drug overdoes, and similar information, concerning Annemarie Catlett (aka Annemarie Hedwig Catlett) during the period from January 1, 2010, to September 5, 2014, including without limitation in connection with any drug overdose incident occurring between March 1, 2014 and September 5, 2014.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Hello Ms. Whitmore,

Attached hereto please find a completed and signed public records request per your request. I am setting forth below information I received from Captain Espinoza of the Curry County Sheriff's Office to assist with the request.

The Curry County Sheriff located the records in our files and the information was City of Gold Beach contacts and cases. The Sheriff does have 2 separate bookings listed for Annemarie Catlett during the time frame requested. The Sheriff indicated the" other records" will more than likely need to be released by the Gold Beach Police Department.

Those bookings are in addition to the drug overdose 911 call and incident report occurring in the first half of 2014.

I have pulled everything we have regarding your records request for Catlett, Annemarie.

You have asked for the cost, it is as follows:

$25.00 labor

$6.50 copy total (.25 cents per page)

$75.00 (approx. attorney review fee's)

$106.50 total estimated cost

Once we received the $106.50, we will process your request. Unfortunately, we do not email police reports for security/hacking reasons. We would be happy to fax them to you, or mail them to you. Please indicate which method you prefer and supply that information to us.

Any 911 recording or CAD's (computer aided dispatch) logs are proprietary property of the Sheriff's Office and we do not disseminate for other agencies.

Hi Gail,

Thank you for documents. I did not find the incident report for Ms. Catlet's valium overdoes that occurred during period from March 1, 2014 and July 4, 2014. The incident involved a 911 call. Officer Tracy Wood and a sheriff's deputy made the call to Vizcaino Court home and an ambulance was summoned which transported Ms. Catlett to the Gold Beach Hospital where her stomach was pumped and she was held overnight. Do you now if there is an incident report on this?

Hi Robert,

I double checked thinking maybe I missed one, but I've sent you everything we had. We don't normally do reports on ambulance/medical calls unless there is a criminal component. Sgt. Wood and the deputy may have just been there as cover for the ambulance crew for safety?

Sgt. Wood will be back Monday and I will check with him and let you know!

We don't have access to the Sherriff's 911 calls.

I wanted to let you know I think Chief Andrews was aware of the overdose incident I am requesting records for as we met when he looked in to check on Annemarie 's welfare while I was at her bedside in the hospital. If you speak with him, please tell him that I appreciated his kindness, as I'm sure Annemarie did as well. He is a good man.

Sorry for the delay in responding. I spoke with Sgt. Wood and Chief Andrews and there are no reports for the 911 incident. Sgt. Wood and the deputy were there just as cover for the EMT's on scene.
