Pursuant to the California's Sunshine Amendment (Cal. Const. Art. I, § 3(b)), I hereby request the following records:
All police dispatch logs and/or investigation reports generated since January 1, 2011 pertaining to any complaints or incidents that occurred at 225 W Broadway Suite 420, Glendale, CA 91204. This location is the regional headquarters for a drug and alcohol rehab network that currently operates under the name "Fresh Start" but has also advertised their services under the following alternative business names:
Narconon Fresh Start
Narconon Southern California
Narconon Drug Rehabilitation Program
Fresh Start Drug and Alcohol Rehabilitation
Fresh Start Centers
I also request that, if appropriate, fees be waived as I believe this request is in the public interest due to the numerous civil lawsuits currently pending against this controversial Scientology-based drug rehab franchise for allegations of negligence, fraud, breach of contract, etc. Disclosure of the records I am seeking will greatly aid the public's understanding of what is happening at this non-profit organization, and how public service agencies operate when dealing with such a facility. Additionally, requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

This is in response to your request for records, which was received by this office on October 12, 2015. You have requested dispatch logs and/or investigation reports generated since January 1, 2011 pertaining to any complaints or incidents that occurred at 225 W. Broadway Suite #420. I have attached a print-out of the call summary responsive to your request. I have also enclosed a list of the Glendale Police Department call type codes and descriptions.
