Pursuant to the Georgia Open Records Act, I hereby request the following records:
–A list of every hospital, clinic, or other institution that provides sexual assault forensic exams administered by a certified Sexual Assault Nurse Examiner (SANE).
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 3 business days, as the statute requires.

Pursuant to the Georgia Open Records Act, I hereby request the following records:

I have reviewed your Open Records Request and regret to inform you that the Department of Human Services (DHS) is not the Custodian of the records you are requesting.
After 2009, the Department of Human Resources ceased to exist and the Office of Regulatory Services is no longer under the Department of Human Services. The Department of Public Health may be the Custodian of these records.

The Georgia Department of Human Services received your original request on October 18, 2017. We responded on October 19, 2017, noting that after 2009, the Department of Human Resources ceased to exist and the Office of Regulatory Services is no longer under the Department of Human Services.
