Pursuant to the Virginia Freedom of Information Act, I hereby request the following records:
- A copy of any data release consent forms parents or students are asked to sign
- Any current policy documents or guidelines governing student data usage
- Any amendments made to those forms from 2015-2019
- Documents outlining any third-party programs or applications collecting student data
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

The FOIA request for Scott County School District regarding data release forms and policy documents governing data are all inclusive on our website as follows: www.scottschools.com<http://www.scottschools.com> If any of this information that you find is insufficient, please notify me and I will respond with copying charges for hard copies. Our policy manual is updated with amendments to policy each year.

This is the second attempt to satisfy the request from our School District. After speaking with our Attorney, the information you are requesting is on the website-www.scottschools.com. The amendments to these student data forms are made to the website each year and the policy manual can be found on the website as well. All the forms you are requesting can be found there. Also, please advise, if this is not sufficient, and we will provide the cost of paper copies mailed to you and if you are willing to incur that cost.
