Pursuant to Florida's Sunshine Law (Fla. Stat. secs. 119.01 to 119.15 (1995)), I hereby request the following records:
Any and all physical force records or Levels of Resistance reports from January 1, 2010 through December 31, 2016 at Dade Correctional Institution. Such reports should contain a statement of the force used and the circumstances under which it was used, as well as the date and area in which the use of force occurred.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I would request your response within ten (10) business days.

Hello Ms. Beard,
Thanks so much for your letter. Unfortunately, I think there may be an error on the invoice. It seems that the charge for 6.625 hours of work (that is, 26.5 units of ¼ hours) at a rate of $14.10 per hour should be $93.41. That combined with 800 pages at 15 cents a page ($120) would total $213.41. Could you please provide a new invoice reflecting this fee? Also, could you please provide an estimate for this request if the materials provide were limited to those from 2012? If there is some way that we might provide a cd, USB drive, Dropbox link, or other electronic option that could reduce the necessity of paper copies, that would be greatly appreciated as well.

Please find the attached amended DC1-201 from the OIG, clarifying that the time estimated to review the records is 26.5 hours. The reference to quarter hours on the form does not literally mean we have to present the time in quarter hours.
