Pursuant to Florida's Sunshine Law (Fla. Stat. secs. 119.01 to 119.15 (1995)), I hereby request the following records:
A copy of any databases that contain, track, or otherwise indicate settled malpractice cases or allegations, medical license revocations or suspensions, and other actions or proposed actions taken against medical professionals by the state that could potentially impact the ability of medical professionals to operate within the state of Florida lawfully.
I request versions of this database that were created or updated between the year 2008 and the date that this request is processed.
One such database was referenced in the following Department of Justice report:

I believe other, related databases are likely to exist.
I also request that, if appropriate, fees be waived as I believe this request is in the public interest. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

For Florida Department of Health disciplinary actions please see the
Florida Department of Health Medical Quality Assurance (MQA) Final Order
and Emergency Action online database which can be accessed at the
following link: http://ww2.doh.state.fl.us/FinalOrderNet/Default.aspx.
For ease of use, you may want to set the Case Status Start Date to
01/01/2008 and set the Case Status End Date to the present date. It is
recommended not to set any of the other parameters in order to receive
the full set of data, which you will be able to then sort and filter as
desired.
The Florida Department of Financial Services Office of Insurance
Regulation collects medical malpractice data. The Office of Insurance
Regulation maintains a searchable online database of closed professional
liability claims. Because of the large amount of data and a limited
search result to 1,000 records at a time, it may be helpful to break up
the database query into smaller increments of time. The database can be
accessed at the following link:
