Copy of electronics waste disposal and/or electronics waste recycling contracts for 2014 & 2015 for all School District facilities.

Copy of hard drive shredding and/or hard drive disposal contracts for 2014 & 2015 for all School District facilities.

Copy of universal waste disposal, including florescent light bulbs and batteries, contracts for for 2014 & 2015 for all School District facilities.

Copy of all procurement records of light bulbs, including Bills of Lading, Shipping Documents and Invoices for 2014 & 2015 for all School District facilities.

Copy of all procurement records of batteries, including Bills of Lading, Shipping Documents and Invoices for 2014 & 2015 for all School District facilities.

Copy of all recycling/disposal records of light bulbs, including Bills of Lading, Shipping Documents and Invoices for 2014 & 2015 for all School District facilities.

Copy of all recycling/disposal records of batteries, including Bills of Lading, Shipping Documents and Invoices for 2014 & 2015 for all School District facilities.

Records of electronics waste, including computers, electronics and TVs, recycled for the previous 3 years including destination of material for 2014 & 2015 for all School District facilities.

Copy of School District’s due diligence policy (or equivalent) for handling end of life electronics. Including criteria for selecting an electronics recycler for 2014 & 2015 for all School District facilities.

Copy of School District’s written policy (or equivalent) for complying with Act 108 of 2010 (Covered Device Recycling Act) for 2014 & 2015 for all School District facilities.

Copy of School District’s written policy (or equivalent) for complying with 25 PA Code 261a.5 (b) for 2014 & 2015 for all School District facilities.

List or records of any fines, penalties or warnings received by School District in regards to improper disposal of Universal Waste including light bulbs, batteries and electronics.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
