All form Form 801 - "Payment to Agency Report" for employees of the Department of Conservation Jason Crapo, Joseph Losado, Conrad Fromme, Francisco Avila, and County Supervisor Diane Bergis
The most current building inspection history report for all properties on Sandmound Blvd.
All complaint forms filed against the Department of Conservation and Development between January 1st, 2016 to February 19, 2022. Complaint forms shall be unredacted.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

This email is to acknowledge that the Department of Conservation and Development ("DCD") received your Public Records Act request, dated February 19, 2022, for: (1) all Form 700 and Form 801's filed by or on behalf of Jason Crapo, Joseph Losado, Conrad Fromme, Francisco Avila, and Supervisor Diane Burgis, (2) all complaint forms filed against DCD between January 1, 2016 and February 19, 2022, and (3) "the most current building inspection history report for all properties on Sandmound Blvd." As to this last topic, we will limit our search and response to building inspection history for the last five years (i.e., the period of January 1, 2017 to the date of your request), unless you provide further clarification.

In addition to DCD, your request has been forwarded to the Clerk of the Board of Supervisors.

Going forward, this office will be coordinating the County's response to your request on behalf of DCD and the Clerk of the Board of Supervisors.
