Pursuant to the Connecticut Freedom of Information Act, I hereby request the following records:

Listing of appeals from local health department/district food service related orders appealed to the Connecticut Department of Health from 2018-present. Document details are not part of this request and would be made under separate cover.

I have been unable to locate any relevant data set on the ct.gov website that contains this information. If it exists, I may be able to search for the relevant date this request addresses.

A listing of dates, disposition, and local health department/district as well as subject establishment would satisfy this request.

Please respond in this portal if any clarification is needed.

I also request that, if possible and pursuant to Conn. Gen. Stat. § 1-212(d), fees be waived as I believe this request is in the public interest and not made for commercial gain. The requested documents will be processed by a representative of the news media/press and made available to the public at MuckRock.com.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

The Department of Public Health has transitioned to a new online system (GOVQA) for processing Freedom of Information (FOI) requests.
