Pursuant to the Connecticut Freedom of Information Act ("FOIA"), I hereby request the following records:

This request concerns Connecticut Department of Children and Families' use of the Eckerd Rapid Safety Feedback system. It is supplementary to the request filed by Michael Morisy on September 16, 2016.

I am requesting the following records related to the above program:

Any correspondence between any staff of the Connecticut Department of Children and Families, including information technology staff, and any other organization or outside vendor, including but not limited to Casey Family Programs, Eckerd Kids, and Mindshare Technology, regarding the selection and implementation of the Eckerd Rapid Safety Feedback system. This includes correspondence in the form of emails, email attachments, paper letters, and any other form of correspondence.

I note in particular that in response to the Michael Morisy request of September 16, 2016, the Connecticut Department of Children and Families kindly produced a document with the filename "Final CT ERSF Logic Model 4-26-16." That document noted that one step in the process of implementing the Eckerd Rapid Safety Feedback system was "Development of Mindshare database & data sharing with ongoing support of DCF IT." Thus, as one element of the general correspondence request above, I would like any correspondence related to the development of the Mindshare database and data sharing between the Department of Children and Families and its Information Technology staff and Mindshare Technology.

I also request that, if possible and pursuant to Conn. Gen. Stat. ยง 1-212(d), fees be waived as I believe this request is in the public interest and not made for commercial gain. The requested documents will be processed by a representative of the news media/press and made available to the public at MuckRock.com.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 4 business days, as the statute requires.

The Connecticut Department of Children and Families is in receipt of the supplementary Freedom of Information Request below. Please be advised that staff is researching the request to determine what, if any, records DCF has that are responsive. With respect to your request that fees be waived, it is DCF's practice to not charge requestors for routine copies of documents as these are sent out electronically. However, if the production of any of the records you request require any additional costs to DCF you will first be informed of the cost prior to their being incurred.
