Pursuant to the Connecticut Freedom of Information Act ("FOIA"), I hereby request the following records:
• All CN 9512 RREC Increased Forfeiture forms submitted to all applicable District Administrators from January 1, 2015 through June 30, 2015.
I also request that, if possible and pursuant to Conn. Gen. Stat. § 1-212(d), fees be waived as I believe this request is in the public interest and not made for commercial gain. The requested documents will be processed by a representative of the news media/press and made available to the public at MuckRock.com.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 4 business days, as the statute requires.

Hello Mr. Washington,
Thank you for your message. May I please receive clarification regarding the lack of materials responsive to this request? This request is based on DOC Directive 9.5: "Any inmate found guilty of an offense listed in Section 10(D) of this Directive may be subject to an additional forfeiture of RREC based on the severity of the offense. In cases where the offense is deemed severe, the Hearing Officer shall make a recommendation to the District Administrator to determine if an additional forfeiture of RREC is warranted and postpone the forfeiture of RREC at the time of adjudication. The Hearing Officer shall make such recommendation by submitting a completed CN 9512 RREC Increased Forfeiture form to the District Administrator for review and decision. The inmate shall be notified of the total amount of any RREC forfeited that may exceed the listed amounts established in Section 10(D)of this Directive by receiving a copy of the completed CN 9512 RREC Increased Forfeiture form within 15 business days of the hearing from the District Administrator."
Could you please clarify whether the lack of responsive documents is due to the lack of any recommendations for additional forfeiture of RREC or is it a matter of record storage or distribution?

I received word from the Disciplinary Hearing Officers that they have not used this form within the timeframe in which you requested. I cannot speak on why they have not used it. I only search for the records. Sorry I could not be any more assistance to you.
