Pursuant to the Connecticut Freedom of Information Act ("FOIA"), I hereby request the following records:

All memos, talking points and/or emails that reference Connecticut Insurance Commissioner Katharine Wade. This includes memos, talking points and/or emails about the advisory opinion issued by Brian O'Dowd on 2/26/16 telling Commissioner Wade that "Given that there are no Cigna matters pending before you...you are therefore not required to take any official action concerning Cigna."

I hereby request such memos, talking points and/or emails from 1/1/15 to the present.

If some of these documents are exempt from disclosure, please provide me with at least a partial response, providing me the documents that are not exempt.

I also request that, if possible and pursuant to Conn. Gen. Stat. § 1-212(d), fees be waived as I believe this request is in the public interest and not made for commercial gain. The requested documents will be processed by a representative of the news media/press and made available to the public at MuckRock.com.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 4 business days, as the statute requires.

As promised, the Office of State Ethics has compiled records responsive to your request of June 3, 2016. Unfortunately, the records are too voluminous to be attached to emails. The most practical and low-cost method for providing you with the records is for our Information Technology staff to burn the records to a disk. The cost of a replacement disk is $1.00. The disk is now ready, and you can choose either to pick up the disk at our office, or let us know whether the address you provided on June 3, 2016 is the appropriate mailing address and we will be glad to mail the disk to you. (Please note that if you choose the mail option, you will be requested to mail a check in the amount of $1.00 to cover the cost of the media.)

Please let me know whether you prefer to pick up the disk, or whether you would like us to mail it to your attention. In the case of mailing, please also provide us with your current mail address.

Dear Mr. Sirota:

Thank you for following up so promptly regarding the data breach. As you discussed yesterday on the phone with Executive Director Carol Carson, this office, in fulfilling your Freedom of Information Act request (described below) inadvertently disclosed to you on a disc the social security #s of Insurance Commissioner Katharine Wade and Superior Court Judge Alice Bruno. During that conversation with Carol you mentioned that you observed the social security #s and that you have not further disseminated this information. I recognize and appreciate that you are familiar with principles of confidentiality and privacy, as well as professional ethical duties. I am asking you to: a) destroy the email attachment containing the social security numbers and b) to confirm by certifying to this below.

The email attachment in question is located on the disc within the “TJ” documents. The “TJ” documents consists of a June 9 2016 email (sent at 11:41a.m. from TJK Jones to Buxo and copied to Housen) with numerous attachments-the attachment at issue is titled “FW: Faxes For You (234KB)”- it contains 2 .pdf letters, each with a social security number.

Kindly check the box below, and return the Certification, signed and dated to me at your earliest convenience.
