Pursuant to the New Hampshire Right to Know Law, I hereby request the following records:

**Unemployment Insurance**
1. Total state unemployment insurance costs, by Federal Fiscal Year, for the years 2015-2020 (through and including the date of this request). Please note that the Federal Fiscal Year begins October 1 and ends September 30 of the following calendar year. Please break costs down by Federal Fiscal Year. If possible, please further break them down by reason for separation (e.g., laid off, fired, constructive discharge, family caregiving-related voluntary quit, employer-initiated change in work location, etc.). If the total state costs are different than the total amount paid to beneficiaries each year, please provide separate totals for 'total costs' and 'total amount paid to beneficiaries.'

2. Please provide ineligibility determination tallies for each 'reason for separation' category. For example, of the total of xyz# of people who applied for benefits because they were laid off, xyz# of people were found to be ineligible for benefits.

**Workers' Compensation Insurance**
3. Total state worker's compensation insurance costs, by Federal Fiscal Year, for the years 2015-2020 (through and including the date of this request).

4. Total state worker's compensation insurance costs, by Federal Fiscal Year, for the years 2015-2020 (through and including the date of this request) broken down by mental-mental claims and mental-physical claims. In other words, claims with a mental or psychological cause that then led to mental harm or physical harm.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.
