Seeking a certified copy of the one-page, allegedly completed, required "Final Disposition Form" known as "Form 1.998"
(and posted online at the court site here: under "Civil Procedures Forms and Instructions" )

for the following matter -- a Sunshine Violations lawsuit filed in Collier County Court as reported by this newspaper article:
