I find your response very odd -- since the form states right on it that this form is required to be filed under FL Statute 25.075.

Upon review of the subject statute, I am seeing the law says the court clerk is guilty of misfeasance if this form is "willfully" not filed:

25.075 Uniform case reporting system.—
(1) The Supreme Court shall develop a uniform case reporting system, including a uniform means of reporting categories of cases, time required in the disposition of cases, and manner of disposition of cases.
(2) If any clerk willfully fails to report to the Supreme Court as directed by the court, the clerk shall be guilty of misfeasance in office.

Consequently, it seems to me that you, a lawyer, upon discovering the form was not filed, should have notified the court clerk and he should have filed it.

But you didn't do that. Instead, it seems to me this form was "willfully" not filed.

I can't help but wonder if the omission of this form is a tip-off that the subject civil complaint is actually -- bogus and does not officially exist. (And, therefore, no form is filed.)

That is what it looks like to me.
