To Collier Court Judge Cynthia A. Pivacek or her assistant Jodi (named online here:

Pursuant to Florida's Sunshine Law (Fla. Stat. secs. 119.01 to 119.15 (1995)), I hereby request the following records from Judge Pivacek's files:
(1) a computerized list from Judge Pivacek or her assistant Jodi of the titles of new cases assigned to Judge Pivacek in April 2009 and May 2009.
(2) a computerized list from Judge Pivacek or her assistant Jodi of the titles of cases for which final orders were signed by Judge Pivacek in June 2009 and July 2009.
Although I am not required to do so, I am stating herein the reason I am making this public records request to Judge Pivacek:
I am trying to determine if Collier Court Clerk Dwight Brock actually assigned to Judge Pivacek a certain case in April 2009, which he claims he did, but I believe that complaint (which I filed via an attorney I now believe is corrupt) was actually never served -- and was never assigned to any judge.
If any the judge had this case, I now believe that judge was guilty of Neglect of Duty for failing to:
schedule a hearing, or, as required by FL statute, schedule a required an "accelerated hearing" as this civil complaint involved a Sunshine Public Records violation of a public school district superintendent (who a year later in 2010, was not renewed by a new school board majority, due to a Sunshine Public Records violation cited by the chair -- exactly what I had already alleged and proved in my 2009 complaint, but my constitutional rights to a hearing and to be heard were violated because there was never any hearing scheduled.
However, right now I do not believe Judge Pivacek ever actually received this case, and I don't believe any judge was assigned this complaint, as I believe the court clerk Dwight Brock was colluding with corrupt politicians directing this criminal violation of Sunshine Law.
I have reason to believe there may be a forgery of Judge Pivacek's signature on a court order in June 2009, and Judge Pivacek was not the one who signed this order, which has only been shown to me for the first time this year, 2015, via MuckRock -- six years after this complaint was ordered closed by this forgery of a judge's signature on an order, without a required accelerated hearing scheduled for this matter.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

I believe the court calendar is required by FL Rules of Judicial Administration to be kept for ten years. So, the record should exist.
However, I already withdrew this request because I have found the Collier Court is too corrupt to deal with.
So, the status was already withdrawn on this Oct 2015 request.
