Pursuant to the Illinois Freedom of Information Act., I hereby request the following records:
Records related to "warnings" and "notice of discontinuation of refuse & recycling collection service" issued in the 26th ward on 12/24/2020. Records and communications including, but not limited to:
-the address for each building that received a warning or notice
-the name of the person issuing each warning or notice
-photos or other supporting documentation
-source for notice or warning (resident, streets and sanitation official, crew, etc and names)
Also, emails between Adam Corona and Edward Fernandez between 12/1/2020 and 12/26/2020.
Also, department policy for issuing "warnings" and "notice of discontinuation of refuse & recycling collection service."
Also, communications to and from Adam Corona's email including the word "police" between 10/1/2017-10/31/2020.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
