FOIA officers,

We are seeking the following information that should be maintained by OEMC

Pursuant to the Illinois Freedom of Information Act (5 ILCS 140/1 to 11), we hereby request the following records:

At or around 5:45pm on February 10th officers responded to a call near 3944 N. Western Ave at a CVS before a police shooting of Michele Robey. We request all scanner traffic from District 19 for this call. Additionally there should be a POD (aka blue light camera) at or near by Addison and Irving Park which may have captured footage of officers responding to that call. We request the POD footage at that time and date as well

In the event that there are fees, we would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Please be aware that the following request is being requested as part of a media/journalistic inquiry.

Dear Mr. Freddy Martinez:

Please see the attached list City of Chicago Departments and the records they maintain which can also be found on our City of Chicago website, www.cityofchicago.org/foia. Please be advised we are processing your request which is logged under FOIA number FA17-0252, however, the first paragraph of the attached list indicates the requirements to submit a FOIA. Specifically, a name, mailing address, and daytime phone number of the requester.

Your request indicates you, Freddy Martinez, are making this request as part of a media/journalistic inquiry. However, you later note "this request is not filed by a MuckRock staff member, but is being sent through MuckRock in order to better track, share and manage public records." You further state that "requests might be returned as undeliverable if addressed in the name of the requester". If "Freddy Martinez" does not work for MuckRock but is the requester, "Freddy Martinez's" information is needed to complete the request including a address and daytime phone number. If MuckRock is the requester, again, a daytime phone number is required.

As I'm sure you can see, this request is very confusing. As well, the request is incomplete. Please clarify who the requester is. If it is not Freddy Martinez, and rather, MuckRock News, in either case, a daytime phone number is required.
