A copy of all Observation Van Worksheet Logs (template: http://directives.chicagopolice.org/forms/CPD-21.959.pdf) from January 1st, 2017 to the date of this request.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

On April 26, 2018, I requested documents under the Freedom of Information Act. My request was assigned the following identification number: P446995. On May 1, 2018, I received a response to my request in a letter signed by FOIA Officer Landon Karr, Chicago Department of Police. I appeal the "no responsive records" result of my request. A copy of my FOIA request and the agency determination which is the subject of this appeal is attached for your convenience.
