Elevator inspections at 444 N Wabash

To Whom It May Concern:

Pursuant to the Illinois Freedom of Information Act (5 ILCS 140/1 to 11), I hereby request the following records:

The last five inspection reports received by the Department of Buildings for the elevator at 444 N Wabash.

The text of any rule or regulation that states how often building owners must submit inspection reports to the Department of Buildings.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

My apologies: I would like to add one other part to my request for elevator inspection reports for the elevator at 444 N Wabash. I would like the last five inspection reports whether they were submitted by the building owners or whether they were performed by the Department of Buildings. And again, I would like the text of any rule or regulation that states how often building owners must submit inspection reports to the Department of Buildings.
