David Toles Arrests

To Whom It May Concern:

Pursuant to the Illinois Freedom of Information Act (5 ILCS 140/1 to 11), I hereby request the following records:

Every arrest report on file for Mr. David Toles (date of birth 6/20/65). Please also provide documentation sufficient to show all officers involved with each arrest.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Dear Ms. Brown,

Due to poor copying techniques, please create and send a new copy of the same documents I requested in FOIA P058122.

If you review the documents you sent me, you will see that many of the pages (mostly pages 4 through 13) are dark and blurry as the result of poor copying techniques. Page 9 even has an upturned corner which obscures relevant information I requested. Please correct this copying error and provide a clearer version of the documents.
