To Whom It May Concern:
Pursuant to the Illinois Freedom of Information Act (5 ILCS 140/1 to 11), I hereby request the following records:
Every arrest report on file for Mr. David Toles (date of birth 6/20/65). Please also provide documentation sufficient to show all officers involved with each arrest.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

I apologize for my late response in regards to the request for an extension. I hope in the time since March 2nd, the date of your last response, you've had time to consult with the necessary public bodies. Given that more than five days have passed since your request for a five-day extension, please provide the documents in the next day or two.

Dear Ms. Brown,
Due to poor copying techniques, please create and send a new copy of the same documents I requested in FOIA P058122.
If you review the documents you sent me, you will see that many of the pages (mostly pages 4 through 13) are dark and blurry as the result of poor copying techniques. Page 9 even has an upturned corner which obscures relevant information I requested. Please correct this copying error and provide a clearer version of the documents.
The files that you requested are older files which are kept on microfilm, I provided you what was provided to me. These are the best copies that are available due to the documents being stored on microfilm and not stored in paper copy.
