Chicago Police Report: Shooting of Michael Moss Jr.

Pursuant to the Illinois Freedom of Information Act (5 ILCS 140/1 to 11), I hereby request the following records:

The police report for the shooting of Michael Moss Jr., who was shot near the 4800 block of South Ada St in Chicago on the afternoon of December 19, 2016. The victim's date of birth is 11/29/2001 and the case RD# HZ557437.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
