Pursuant to the Illinois Freedom of Information Act (5 ILCS 140/1 to 11), I hereby request the following records:
Any and all internal reports, audio, video, handwritten and/or electronic reports, biometrics, aerial surveillance documentation, either by drone or plane, covert phone and or home audio, car, computer and interior property surveillance collected by the Chicago Police Department on E. Coady and 1308 W. Waveland, Unit 1, Chicago, Illinois.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

I am seeking any and all documentation, handwritten and/or electronic notes, memos, emails, reports, profiles, directives, audio tapes, video tapes, Stingray smart phone app or aerially collected data pertaining to surveillance of 1308-1 W. Waveland, Chicago, 60613. Please acknowledge this error on your part and my request.

This public records request was initially submitted by E. Coady on November 5, 2015. On November 17, we received an acknowledgement letter for request 15-6448, which was related to a Mr. Goins. That is not the request which was made in this particular instance. The request in question refers to:
