Pursuant to the Illinois Freedom of Information Act., I hereby request the following records:

List of addresses in Chicago, in an electronic data table if possible, that have had their lead service lines replaced through the:

-Equity Lead Service Line Replacement program

-Homeowner-Initiated Replacement Program

-Break or Leak in Water Line pursuant to state law mandate beginning January 1, 2023

-Through Department of Water Management routine maintenance and repair operations for all years available up to 2023.

If addresses are not available due to privacy reasons, please provide the list anonymized at the block level.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
