Pursuant to the Illinois Freedom of Information Act (5 ILCS 140/1 to 11), I hereby request the following records:

Seeking 911 audio recordings placed between Januaury 1, 2014 and December 31, 2014, from 1308-1 W. Waveland, Chicago as well as any internal communications between City of Chicago officials, employees or departments about said 911 telephone calls.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
