Copies of the attachment sheets, summary digest report, and any other final disposition documentation (e.g., an Administrative Closure order, mediation agreement, grievance decision or arbitration award) for each of the following CR investigations. If possible, please provide all files associated with each individual CR together in one PDF file per CR number.

Your request has been forwarded to the Bureau of Internal Affairs for review. This request might be burdensome under 5 ILCS 140/3(g) and may require alteration to make it more manageable. There is need for consultation, which shall be concluded with all practicable speed, with another public body or among two or more components of a public body having a substantial interest in the determination or in the subject matter of the request. It is therefore necessary to extend the time for response by five business days to November 18, 2019, as permitted by 5 ILCS 140/3(e).
