Pursuant to the Virginia Freedom of Information Act, I hereby request the following records:
Any forms, telematics reports or driver reports for Charlottesville Police Department cruiser with Tag #157-339L on May 7, 2019. Please include any FR-300P forms, VML Insurance claims reports, Supervisor's Incident reports and any subsequent findings from the Accident Review Board indicating either "Avoidable" or "Unavoidable". In the event that the documents contain personal identifying information which cannot be released, I ask that only the unreleasable information be redacted and all other releasable information be provided per the precedent set by Jacobsen & Landers v. City of Charlottesville, et al. The documents may be in the custody of the current Accident Review Board chairperson (see attached file).

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

Tom - The documents related to the attached FOIA request are being entirely withheld pursuant to Code of Virginia Section 2.2-3706 (B)(1). This information is a part of an investigative file and is exempt from release under FOIA. CPD is withholding approximately (8) documents from release.
