Hi Sam,
Most of this information is available on federal OSHA's website. You can search by NAICS industry code 493110 for information on previous Cal/OSHA fatality, accident and complaint investigations in the general warehousing industry including Amazon Fulfillment facilities. The accident investigations are based on notifications of serious injuries, which are those that result in hospitalization.

Regarding copies of the 300 log forms, Cal/OSHA only has a copy of the forms if they were requested in an investigation. They do not exist in one central place so you have to ask for an investigation and we can check if there is a 300A summary form that can be disclosed.

Here are the search results for 2018 in California: https://www.osha.gov/pls/imis/industry.search?p_logger=1&sic=&naics=493110&State=CA&officetype=All&Office=All&endmonth=01&endday=01&endyear=2018&startmonth=12&startday=31&startyear=2019&owner=&scope=&FedAgnCode=
