From the Division of Industrial Relations, I would like the following details:

The number of fatal injuries in the Warehousing and storage sector for 2018
The number of non-fatal injuries in the Warehousing and storage sector for 2018

The number of fatal injuries reported at Amazon Fulfillment Centers for 2018
The number of non-fatal injuries reported at Amazon Fulfillment Centers for 2018

If possible, a log of workplace related injuries reported at Amazon Fulfillment Centers for 2017-2018 (similar to an OHSA form 300)

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Hi Sam,
Most of this information is available on federal OSHA's website. You can search by NAICS industry code 493110 for information on previous Cal/OSHA fatality, accident and complaint investigations in the general warehousing industry including Amazon Fulfillment facilities. The accident investigations are based on notifications of serious injuries, which are those that result in hospitalization.

Regarding copies of the 300 log forms, Cal/OSHA only has a copy of the forms if they were requested in an investigation. They do not exist in one central place so you have to ask for an investigation and we can check if there is a 300A summary form that can be disclosed.
