To Whom It May Concern:
Pursuant to Florida's Sunshine Law (Fla. Stat. secs. 119.01 to 119.15 (1995)), I hereby request the following records:identification of the objects on the telephone pole line/pole.
There is a telephone pole positioned in the NE corner of my back yard. Years ago, FPL identified the address assigned to this pole as 1100 S. 24th Terrace #OL, Hollywood; which is different from my house address.
There are two protruding black like items; somewhat attached and branching away from each other (pointing in different directions). Please identify these objects as they are attached on the telephone pole / telephone pole line behind my home.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Please, clarify your request: I hereby request the following records:identification of the objects on the telephone pole line/pole.
What record is being requested?
Are you saying you filed a police report regarding objects on a telephone line and you need a copy of it?
Also, that area is patrolled by the Hollywood Police Department. If the report was filed with Hollywood PD, you would need to contact them directly.
