Pursuant to the New Jersey Open Public Records Act, I hereby request the following records:
I would like to request any and all correspondence, interviews, and notes documenting calls, in regards to the cyclospora outbreak from June 1, 2019 to July 30, 2019, including but not limited to my own records.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 7 business days, as the statute requires.

The Clerk's Office has received your OPRA request.
The building is currently closed to the public.
Unfortunately, due to the COVID-19 situation, all OPRA requests will have an automatic thirty day extension at minimum.
If we can process this request before the extension time is up we most certainly will.
If you receive the information you are looking for prior to receiving it through the OPRA process, please let me know so we can cancel this request.

The Health Department has your request.
As stated in my prior email - unfortunately, due to the COVID-19 situation, all OPRA requests will have an automatic thirty day extension at minimum.
Your request is actually OPRA #20-409 and the time frame for a response could be between the Due Date 6.4.20 and or the Extension Due Date 7.8.20.
I have carbon copied the Health Department on this email.

Hello. I apologize for the delay. We have found no records relevant to OPRA Request #20-409, and this request is now closed. According to our Health Officer, you would need to direct your request to the New Jersey Department of Health. Although you may have filed a report with this office, staff would have used that report to enter the information into a State database, and subsequently destroyed the questionnaire to protect your privacy. The State may be able to provide you with the information.
