Pursuant to the California Public Records Act (California Government Code §§ 6250), I hereby request the following records:

All information, including without limitation all records, documents, incident reports, traffic tickets, traffic stops, infractions, violations, arrests, welfare calls, suicide attempts, complaints, victim reports, criminal charges, domestic violence calls, 911 calls, and similar information, concerning Annemarie Catlett (aka Annemarie Hedwig Catlett) during the period from January 1, 1995 to the present, including without limitation in connection with any welfare call for threatening self-harm or harm to others.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
