Pursuant to the Illinois Freedom of Information Act (5 ILCS 140/1 to 11), I hereby request the following records:
- A list of all new employees hired between January 1, 2011 and December 9, 2015 including their date of hire, titles by year, and salaries by year.
- A list of all new positions created by the City of Berwyn during the same time period, including the titles and the job descriptions of those new positions, and salaries by year.
- Any document that describes the hiring practices of the City of Berwyn, including any hiring freeze, from January 1, 2011 and December 9, 2015.
- Any document or law that regards nepotism or in any way limits hiring practices based on the relationship of the potential hire-ee to current members of city government.
- All emails to or from city accounts that contains the search term “Jynette”. (Quotes not included in the search term.)
- Any public disciplinary files of Jynette Ayala that are not subject to exemption or redaction as per Illinois law.
- Job titles, job descriptions/duties of each of these positions, held by Ms. Ayala, as well as salaries, and dates of employment in those positions.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

Mr. Smith, Attached you will find your FOIA request, our #16-037 and a notice of extension. I apologize, once again my office did not receive this back in December of 2015. The e-mail address that it was sent from was identified as a spam e-mail and ended up in my spam box of 2-2-16, (see attached screen shot) as it may have back in December, as well and had gone undetected. You should receive a response from the City of Berwyn on or before 2-17-16.
