From: R. M. Seibert

To Whom It May Concern:

Pursuant to Michigan's Freedom of Information Act ("FOIA"), I hereby request the following records:

All law enforcement call logs and incident reports generated since March 1, 2002 pertaining to any events that occurred at the Narconon drug rehabilitation located at 216 Mary's Lake Rd., Battle Creek, MI 49017. This facility is currently registered with the state as "A Forever Recovery" but has also been known to operate under the business name "Narconon Stone Hawk."

I also request that, if appropriate, fees be waived as I believe this request is in the public interest due to the numerous complaints filed with the County Health Department and Michigan Attorney General against this controversial Scientology-based rehab. Disclosure of the records I am seeking will greatly aid the public's understanding of what is happening at this non-profit organization, and how public service agencies operate when dealing with such a facility. For more information, please see:

Additionally, the requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

Sincerely,

Ms. R. M. Seibert
