Pursuant to the Massachusetts Public Records Law, I hereby request the following records:

Property receipt and or any files, documents, warrants or records regarding the computer taken from 797 Carpenter Road without providing the owner Jose Oquendo a Search Warrant, receipt or any documentation with regard to its seizure.

Property receipt and or any files, documents, warrants or records regarding the cell phone belonging to Ashley Oquendo which has landed in the possession of the Athol Police Department without providing the owner Ashley Oquendo a Search Warrant, receipt or any documentation with regard to its seizure, possibly including Body Cam footage of the Athol Police Department advising Ashley Oquendo she cannot report said phone stolen and that she was to re-activate it immediately.

Any and all complete files, records, images, recordings or any form of document with regard to the property at 797 Carpenter Road, Athol MA 01331 from October 1, 2020 to present. This should include any complaints made to the Building Department or any town agency.

Any and all complete files, records, images, recordings or any form of document with regard to the inspection and certificate of occupancy supplied for 797 Carpenter Road, Athol MA 01331 without a proper inspection being done and with many on-going property and code violations.

Any and all complete files, records, images, recordings or any form of document with regard to Jose Oquendo formerly from 797 Carpenter Road, Athol MA 01331 from October 1, 2020 to present. This should include any record of the Athol Police Department's refusal to accept a police report for abuse and battery, refusal to provide a Warrant for the seizure of above referenced Computer, refusal to file a theft report for stolen medication. This should include phone conversations made when Jose Oquendo contacted the Athol Police Department as the lines are said to be recorded. This should also include body cam footage from October 12th 2020 when Jose Oquendo was removed from the property at 797 Carpenter Road.

Any and all complete files, records, images, recordings or any form of document with regard to Nicole Oquendo 797 Carpenter Road, Athol MA 01331 from October 1, 2020 to present. This should include the original affidavit given by Nicole Oquendo on October 12, 2020 which the Athol Police Department has thus far refused to provide.

Any and all complete files, records, images, recordings or any form of document with regard to Ashley Oquendo 797 Carpenter Road, Athol MA 01331 from October 1, 2020 to present. This should include Ashley Oquendo's initial police report made on October 12th 2020 and any follow up work done with regard to said complaint. This should include the refusal to provide a Wellness check on Mathew and Samantha Oquendo as well as the theft of Ashley's dog and refusal to do a wellness check on said dog Whiskey. This should include phone conversations made with Ashley Oquendo. This should include any supporting documentation by which Ashley Oquendo has been advised by the Athol Police Department that she is not allowed to enter the property at 797 Carpenter Road although she pays rent and utilities there and has not been served nor evicted.

I also request that, if appropriate, fees be waived as we believe this request is in the public interest, as suggested but not stipulated by the recommendations of the Massachusetts Supervisor of Public Records. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.

I expect the request to be filled in an accessible format, including for screen readers, which provide text-to-speech for persons unable to read print. Files that are not accessible to screen readers include, for example, .pdf image files as well as physical documents.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
