Pursuant to the Arizona Public Records, I hereby request the following records:
-Written complaints submitted to the Regional Behavioral Health Authority (RBHA) customer service department from January 2017 to the date this request is processed
-Written concerns about quality of care emailed to the Arizona Health Care Cost Containment System (ACCCS) Clinical Resolution email address (CQM@azahcccs.gov) from January 2017 to the date this request is processed
-Submitted grievances/requests for investigation by health plan January 2017 to the date this request is processed
Please provide materials on a rolling basis, as they become available. We request that the requested complaints please be made available to us as soon as possible.

Arizona Revised Statute 36-2917 mandates that concerns that are sent via the CQM@azahcccs.gov<mailto:CQM@azahcccs.gov> email address are confidential and may not be disclosed. Any review and investigation of the conduct of licensed health care providers in order to determine whether disciplinary action should be imposed is confidential.
Complaints submitted to RHBA's and health plans are handled at the RHBA's and health plan level and are not elevated to AHCCCS. We have no responsive documents.
AHCCCS has addressed your request and feels this matter is complete and there is no need to address it on an ongoing basis.
