CACF Welfare and Benefits Fund reports

To Whom It May Concern:

Pursuant to the state open records law, Ariz. Rev. Stat. Ann. Secs. 39-121 to 39-122 and 39-128, I hereby request the following records:

The following materials related to the Welfare and Benefits Fund of the Central Arizona Correctional Facility as indicated by Section 11.17 of Contract No. 040176DC:

• All receipts or related materials documenting the transmission of revenues generated from use of the inmate telephone system to the Contractor
• All reports regarding the transmission of income from the sale of commissary goods
• All payouts (for Commissary Manager salary costs, equipment, or any other reasons) made from the W&B Fund
• All major purchases with W&B Funds for which a sealed bidding process was necessary
• All proposed expenditures from the W&B Fund
• All financial reports

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
