Pursuant to the Michigan Freedom of Information Act, I hereby request the following records:
Any open records requests, salary market survey, labor market data from purchased salary surveys as well as custom local area survey results about or done for a Perfusionist or group of Perfusionists in the past 9 years, by the units at Michigan Medicine where perfusionists are employed.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

I am writing in regard to your Freedom of Information Act request sent by U.S. Mail, which was received on December 4, 2018.
Due to the large number of requests currently being processed by this office, it will not be possible to respond to your request within the five-day period accorded by the Michigan Freedom of Information Act. However, under Section 5 (2) (d) of the Act, the University is permitted to extend the deadline for not more than 10 business days beyond the initial due date.
The University will respond to your request on or before January 2, 2019.
