Pursuant to the Alaska Public Records Act, I hereby request the following records:
I am requesting an opportunity to inspect or obtain copies of public records that include an updated list of all Alaska State MEDICAID HEALTHCARE providers and ALL Alaska STATE LICENSED HEALTHCARE PROVIDERS in electronic data set (either CSV, excel or PDF electronic format) to include Physicians (MD, DO), Physician Assistants, Nurses, CRNA, Nurse Practitioners, Midwives, Dentists, Podiatrists, Dental Assistants, Chiropractors, Physical Therapists, Occupational Therapists, Social Workers, Family therapists, Psychologists and so on. All types of practitioners from all dates electronically available would be appreciated. Specific items to be included in data set:
1. Provider name, Provider type (MD, DO, PA, RN, PT, OT, CRNA, DMD, DDS, DPM etc)
2. National Provider Identification (NPI) number
3. Alaska State Healthcare License number and initial date of issuance or expiry (if available/applicable)
4. Education / School/Training, if available
5. Specialty
8. Practice Location
If there are any fees for searching or copying these records, please email me if the cost will exceed $50. However, I would also like to request a waiver of all fees in that the disclosure of the requested information is in the public interest and will contribute significantly to the public’s healthcare transparency, fairness and public awareness.
The law requires that you respond to and fulfill this request "as promptly as possible." If you expect a significant delay in responding to and fulfilling this request, please contact me by phone or via email with information about when I might expect copies or the ability to inspect the requested records.
If you deny any or all of this request, please cite each specific exemption you feel justifies the refusal to release the information and notify me of the appeal procedures available to me under the law.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

Requests of this nature are responded to under the Alaska Public Records Act [APRA] (AS 40.25.100 et seq. and 2 AAC 96.100 et seq.).
Information for all licensees regulated by this division is available to the public for download in Excel format at https://www.commerce.alaska.gov/CBP/Main. That download includes the name of the licensee, type of license, date the license was issued, and the status of the license. This list also includes addresses for the licensees; however, the address listed is not necessarily the practice location for the licensee.
The division does not have a list that includes a National Provider Identification (NPI) number, education and/or school/training, specialty, or practice location and is not required to create such a list in response to an APRA request. Therefore, your request for the information not contained in the list that is available online to the public is denied in accordance with AS 40.25.120 and 2 AAC 96.210(b) and (c). However, you may search individual licenses and may or may not find certain additional information from an individual license search. Additional information can also be requested for individual licensees by filling out a Public Records Request form which is available at https://www.commerce.alaska.gov/web/cbpl/PublicRecordsRequests.aspx.
Division Director Sara Chambers has delegated her authority to me to deny this request. You may immediately appeal this decision by seeking injunctive relief from the Alaska Superior Court. Additionally, you may ask Ms. Chambers for reconsideration. Such a request does not require posting an appeal bond. For more on this process, please review the enclosed regulations, particularly 2 AAC 96.340. If you choose not to pursue injunctive relief in the Alaska Superior Court,
your rights before this division, including requesting reconsideration, will not be adversely affected.
