To Whom It May Concern:

Pursuant to the California Public Records Act, I hereby request the following records:

A record that shows all of the Covid-19 tests reported by Alameda County from March 1, 2020 to present that shows how many tests were administered by The County Health Department and how many were non-County controlled and/or administered: these are from private healthcare providers.

Additionally, if these (county and non-county controlled tests from March 2020 to now) could be broken down by race. For example (using fake numbers):

Kaiser Permanente: 5,876 white tests. 678 Black tests.
ACDPH: 10,876 white tests. 789 Black tests.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 calendar days, as the statute requires.

Sincerely,

Annika Hom
